You can now send secure electronic messages from your computer or mobile device to your named GP. This is via the Patient Access website or app. Your named GP will respond to your message within 2 working days.
We hope that patients will find this a convenient way to access advice and healthcare from the surgery. Please do not use it for urgent medical requests.

We request that all new patients bring in proof of address. For example, this could be in the form of:
All documents must be dated within the last 3 months.
We are unable to accept mobile phone bills as proof of residence.
We will need your NHS number when you register. Your previous GP surgery can provide you with this.
To register with our practice you will be required to complete two registration forms: one is the standard NHS GP registration form (GMS1) and the second is a new patient practice registration form. The forms are found here:
Please print and complete both forms, and bring them to the GP surgery with your ID and proof of address. Alternatively, our reception can provide you with the forms to complete in the surgery.
Once you register, we will likely advise you to book to see our Health Care Assistant for a brief new patient medical check.
You will be registered with an individual doctor at the surgery, but can see any doctor of your choice.
If you are registered with us and move out of our catchment area, we strongly advise you to register with a new GP surgery more local to your new address. You may request to stay registered with us and the senior practice team will decide if it is clinically safe and advisable for you to be registered out of area or not. Their decision is final.
If you live outside of the practice area and wish to be registered with us (e.g. because you work near to the surgery), you may request to be registered with us. The senior practice team will decide if it is clinically safe and advisable for you to be registered out of area or not. Their decision is final.
Any patient who lives out of area can only be registered if they agree that we cannot provide them with a home visit.
If you are temporarily in our area and need to see a GP for an urgent problem, we are able to provide care. Please print and complete the GMS3 form before coming to the surgery. We have the form at reception if you are unable to print.
Please inform us if you move house, or change your telephone/mobile number so we are able to keep your records up to date.
