New Patient Registration
You are able to join our surgery if you live within our catchment area, or even if you live just on the border of our catchment area, on the understanding you will not receive home visits. Click here to see a map of our catchment area.
You can also view the street list to check your street is within our catchment area.
Patients who move outside this area will have to find a new practice.
We register patients on:
- Monday - Friday 10am - 12.00pm
- Monday - Wednesday 3pm - 6pm
- Friday 2pm - 5pm
Please note we are unable to register any children under 5, who are new to the practice, without their immunisation records.

Proof of address
We request that all new patients bring in proof of address. For example, this could be in the form of:
- A utility bill
- Bank statement
- Council tax bill
- A pension or benefits documents
All documents must be dated within the last 3 months
We are unable to accept mobile phone bills as proof of residence. Due to a change in NHS policy, we cannot accept registrations without an NHS number. Please contact your previous GP for this information.
Forms to Complete
To register with our practice you will be required to complete two registration forms: one regarding your personal details (GMS1) and the second regarding any health issues that you may have. You can download them by clicking on their names below:
At this stage we may need to book you an appointment to see our Health Care Assistant so that you can have a basic medical check-up.
You will be registered with an individual doctor at the surgery, but can see any doctor of your choice.
If you are temporarily in our area and need to see a GP for an urgent problem, we are able to provide care. Please complete the GMS3 form below before coming to the surgery.
Change of Details
Please inform us if you move house, or change your telephone/mobile number so we are able to keep your records up to date.
